Basic Criteria:
- Be currently diagnosed and treated for type 2 diabetes (with an A1C between 6.5% and 10.5%)
- Have a history of high blood pressure
- Have heart disease, defined as meeting one of the following:

A history of heart procedures, such as stents or bypass surgery

A prior stroke

Surgery or stents placed in the neck arteries

Poor circulation in the legs, including past procedures, amputation, or ongoing leg pain when walking due to reduced blood flow
- Meet at least one of these additional risk factors:

- Have a eGFR between 30 and 60 mL/min/1.73 m2

- Have a history of polyvascular disease (in addition to meeting one of the criteria for heart disease, as explained above)

- Have a history of atrial fibrillation or atrial flutter

- If female of childbearing potential, are you currently pregnant, breastfeeding, or planning to become pregnant?
- Are you currently receiving treatment with any of the following: apalutamide, avasimibe, carbamazepine, enzalutamide, lumacaftor, mitotane, phenytoin, rifampin, rifapentine, St. John’s wort, triamterene, amiloride, aliskiren?
- Have you ever been diagnosed with or treated for heart failure?
- Do you been diagnosed with stage 4 or 5 kidney disease, or have you had any dialysis procedure or acute kidney injuries in the last 3 months?
- Do you have type 1 diabetes or uncontrolled type 2 diabetes (HbA1c >10.5%)?
- Have you had a stroke, TIA, heart attack, valve implantation/replacement, carotid surgery/angioplasty, or cardiac surgery in the last 3 months?
- Have you previously received an organ or bone marrow transplant, or do you plan to receive an organ transplant within the next 6 months?
- In the past 3 months, have you participated in any other study with an investigational product administered?
Answers Must be Yes
- If you female of childbearing potential, are you willing to use a highly effective form of birth control during the study and for 1 month afterward?
- Are you willing to refrain from smoking/vaping, consuming alcohol, and caffeine-containing food or beverages prior to clinic visits?
