Heart Disease (ASCVD) Self-Scheduler

Basic Criteria: 

• Be 18 years or older

• Have experienced at least one of the following cardiovascular events:  

• • A Heart Attack 

• • A Large Artery Ischemic Stroke within the last year 

• • Revascularization procedure due to symptomatic Peripheral Artery Disease 

• Have at least one of the following conditions/factors: 

• • Type 2 Diabetes 

• • Aged 65 or older 

• • Multivessel Disease 

• • Additional previous Heart Attack 

• • Stage A, B or C Chronic Kidney Disease (CKD) 

• • Lower extremity revascularization due to poor circulation or to treat a large vessel ischemic stroke 

• Be on a stable dose of maximum-tolerated Statin for at least a month

Answers Must Be No: 

• If female of childbearing potential, are you breastfeeding, pregnant, or planning on becoming pregnant? 

• Do you have a coronary revascularization procedure planned in the next 6 months? 

• Do you have uncontrolled high blood pressure? 

• Do you have active liver disease?

• Have you taken cholesterol-lowering medications (such as mipomersen or lomitapide) in the last year?  

• Have you used PCSK-9 inhibitors (such as evolocumab/alirocumab) in the last 3 months? 

• Have you been diagnosed with Class 4 Heart Failure? 

• Are you an organ transplant recipient? 

• Have you ever had inclisiran therapy or do you plan to have it in the future? 

• Have you had cancer in the last 5 years? (Non-melanoma skin cancers and cervical in-situ carcinoma are allowed)

• Do you have HIV, Hepatitis A, or Hepatitis B? 

Answers Must Be Yes 

• If female of childbearing potential and sexually active, are you willing to use a highly effective form of birth control during the study?
