Chronic Heart Failure Self-Scheduler

To qualify, you must

Basic Criteria: 

  • Be 18+
  • Must be diagnosed with Chronic Heart Failure in the past 3 months  

Answers Must Be No: 

  • Are you pregnant or breastfeeding? 
  • Have you been hospitalized in the past month due to acute decompensated heart failure or IV therapy including diuretics, inotropes, or vasodilators? 
  • Have you had a heart attack, stroke, cerebrovascular accident or major surgery in the past 3 months? 
  • Are you a heart transplant recipient, currently awaiting a heart transplant, or have an implanted left ventricular assist device (LVAD)? 
  • Have you had known cardiomyopathy in the past year? 
  • Do you have symptomatic bradycardia, sick sinus syndrome, Mobitz Type II second-degree AV-Block, or third-degree heart block? 
  • Have you had an implanted cardioverter defibrillator (ICD) in the past 3 months? 
  • Do you have chronic lung disease requiring intervention? 
  • Have you had a history of ketoacidosis in the past 5 years? 
  • Do you have Type 1 Diabetes or a history of any other autoimmune causes of diabetes? 
  • Do you have known adrenal insufficiency or Cushing’s syndrome? 
  • Have you had cancer in the past 2 years (except skin, cervix, or prostate cancer)? 

Answers Must Be Yes 

  • Are you willing to use a highly effective form of birth control? 
  • Must be diagnosed with Chronic Heart Failure in the past 3 months 
  • Additional criteria will apply
